The Hookah uses herbs instead of tobacco but originally the eastern folks used a lot stronger stuff. As for your question, the heat from the vapours are cooled down through the water bong, which also removes a lot of the toxins. It is by far a lot safer than inhaling a cigarette. As for marijuana, studies I have read say the weed is safer than tobacco, but I have my suspicions.
